Submit an Intake Request Form

The first step in scheduling an appointment with us is to fill out our appointment request form. By filling out this form, you partner with us to provide vital information that we can utilize to make a compatible match between you and your therapist!

Schedule An Intake Call 

Once your form has been submitted, our Client Care Coordinator will contact you to schedule a short phone or zoom intake call. During this call the Client Care Coordinator will use their knowledge of the therapists on our team and your goals for therapy to recommend a therapist whose skill set and specializations will best suit you.

Once a match has been made, we will get you scheduled for your first session and send you some paperwork to fill out.

We do our best to accommodate our clients' scheduling needs. Please note that after school and weekend times are the most requested times. If you have more flexibility in your schedule, the sooner we'll be able to get you onto the schedule. We are able to provide a school note to excuse them from class. Please list all possible OPEN availability below.
Austin Family Counseling does not accept any form of insurance; we are a private pay practice. This means clients pay the full cost of counseling services at each visit. If you have insurance, you may be able to file a claim with them to seek reimbursement for your out of pocket cost. Although AFC can provide you a superbill to use when filing a claim, AFC can not assist you in working with insurance and there is no guarantee your insurance will provide any reimbursement.
