Submit an Intake Request Form

The first step in scheduling an appointment with us is to fill out our appointment request form. By filling out this form, you partner with us to provide vital information that we can utilize to make a compatible match between you and your therapist!

Schedule An Intake Call 

After you submit an intake form, you will be prompted to schedule a short phone or zoom intake call. During this call, a member of the Client Care Team will use their knowledge of the therapists on our team and your goals for therapy to recommend a therapist whose skill set and specializations will best suit you.

Schedule An Appointment

Once a match has been made, we will get you scheduled for your first session and send you some paperwork to fill out.

AFC is private pay; you pay out of pocket for all services we provide.  If you have insurance, they will not pay us directly for services but you may be able to seek out of network reimbursement for some costs depending on your insurance policy.  Click here for more information.
